A childcare receipt documents payment for daycare, babysitting or after-school care — including the provider’s details, the child’s name, the dates of care, and the amount paid. In many countries, parents need this exact information, including the provider’s tax ID, to claim a dependent-care tax credit. Use the form above to generate one with all the required fields.

Why the Provider’s Tax ID Matters on This Receipt

This is the one receipt type where a missing field can directly cost the customer money: many tax authorities require a childcare provider’s tax identification number before a parent can claim a dependent-care credit or deduction. A daycare center, in-home nanny or babysitter who leaves this off risks parents being unable to claim the credit at all — which is why it has its own dedicated field in this template rather than being buried in a generic notes box.

How to Create a Childcare Receipt

  1. Add the provider’s name and tax ID. This is the field parents will need most — don’t skip it if you have a registered tax number.
  2. Enter the child’s name. Especially important for providers caring for multiple children from the same family.
  3. Record the care dates or billing period. Daily, weekly or monthly — whichever matches how you bill.
  4. Add the rate and total. Enter the daily/weekly rate as a line item; the generator totals it automatically.
  5. Download and provide to the parent. Export as PDF — parents typically keep these for annual tax filing, so a clean, complete copy matters.

What Should a Childcare Receipt Include?

  • Provider name, business name (if applicable), and tax ID
  • Child’s name
  • Care dates or billing period covered
  • Daily/weekly rate and total amount paid
  • Payment method and date received

Who This Template Is For

Licensed daycare centers, in-home nannies, and babysitters who bill parents regularly — as well as parents who need to request a compliant receipt from a provider who doesn’t normally issue one, ahead of filing their taxes.

Parents claiming a dependent-care credit should also check our guide on how long to keep tax records.

Do I need a tax ID number to issue a childcare receipt?

Not to issue a basic receipt, but if the parent intends to claim a dependent-care tax credit, most tax authorities require the provider’s tax ID on the receipt for that claim to be valid. Include it if you have one.

Can I issue one receipt covering a full month of care?

Yes — enter the billing period (e.g. “March 1–31, 2026”) and the total monthly rate as a single line item.

What if I’m an informal/unregistered babysitter?

You can still issue a receipt with your name and the payment details — just leave the tax ID field blank. The parent should check with a tax professional on whether informal care still qualifies for any credit in their situation.

Can this be used for after-school or holiday care programs?

Yes — the same fields apply whether it’s daily daycare, after-school care, or a holiday care program.
